In consideration of the acceptance of this application by Cloud Nine, Inc., I agree to hold harmless the training school, its staff, its owners, and all persons connected to or associated with it, and I do hereby release the same from any and all claims I may have at any time as a result of my participation in classes or activities, and including the following:

• Any loss or injury that may occur directly or indirectly to any person or thing by an act of the dog described above while he/she is on the premises or grounds or in or near any entrance or exit, it being my intention and agreement to assume full responsibility and liability thereof;
• The disappearance or loss of the dog described above by theft, accident, death, or through a connection with the school in any manner, or by any other cause operating direclty or indirectly while the dog and/or I are on or about the premises of Cloud Nine, Inc.
• I certify that the above-described animal is not a wolf or a wolf hybrid.

Please Note:

• Full payment will be due on the first night of class. We accept cash, check or credit cards. Please note: payment in advance secures your spot in a class. Simply mail a check to Cloud Nine Training School for Dogs, 640 11th Ave South, Hopkins, MN 55343 or call Dawn at 952-939-9174 with your credit card information.
• Refunds are not given after you have attended the first class.
• You must clean up after your dog.
• Bitches in season must stay home but owners may attend class.
• Schedule is subject to change.
